Living Will
This is a service of North Carolina State University's North Carolina Cooperative Extension Service. The form is free, and there is no information on filing fees. You will need to get the form notarized after you fill it out. You can also get a copy of this information from Moss.

Duke University Project on Psychiatric Advance Directives
A Psychiatric Advance Directive is a legal document, based on the Living Will, that allows a potential mental health patient to state what treatment(s) s/he is willing (or unwilling) to undergo, what doctor(s) s/he is willing (or unwilling) to be treated by, etc. for hospitals, treatment centers, and so forth. You can also designate an Agent, somebody you personally trust to be on your side during treatment and who can take legal steps to enforce your Directive. This is a necessary document for people who have been abused by doctors, hospitals, and therapies.

MindFreedom International
MindFreedom is an organization of people who have been treated -- and harmed -- by psychiatric methods, as well as a coalition of other such organizations AND is certified by the United Nations as a NGO (non-governmental organization in an advisory capacity). They offer a lot of support to individuals with any mental illness, including protection from forced treatment, and publish "MindFreedom Journal".
